For Pediatric Patients: Scanned copy of the child’s computerized birth certificate. Scanned copy of one parent’s national ID (both sides). For Adult Patients: Scanned copy of the patient’s national ID (both sides) or passport. For All Patients: Two phone numbers (patient or first-degree relative). Medical documents showing the GIT condition (test results, endoscopy reports, medical reports, or CT scans of the digestive system). All documents must clearly display the patient’s full name. Important: All required documents must be complete and clear. The first step is to schedule an appointment at the preparatory clinic (waiting list applies).
